By signing this form, I, for myself and an minor children for whom I am responsible, acknowledge the risk of injury relating to participation in clean up and trail maintenance activities and I agree to release Shenango River Watchers, and any landowner on whose property I am volunteering, from any liability for property damage, injury, dismemberment or death to myself or any minor children, during this activity.

I authorize Shenango River Watchers to photograph me and any minor children for whom I am responsible during this activity and to utilize my and their likeness in its print or digital publications.
